The Shropshire Wellbeing and Navigation Network (SWANN) is a preventative support service for adults aged 18 and over living in Shropshire. It helps people: Stay well Remain independent Feel connected to their community Support is available before problems become more serious and aims to reduce the need for crisis or statutory services.
