Age UK Shropshire Telford & Wrekin’s Home from Hospital service offers up to six weeks assistance to any individual who is either returning home from hospital, or as a preventative service to help individuals remain in their own home, preventing hospital admission.Caring volunteers visit each week for up to two hours, and will collect prescriptions, fetch shopping, take service users shopping, accompany to a doctor, or simply visit for companionship.The service is free, and volunteers, working with other agencies, carers, and/or family members, support and encourage service users to regain confidence and independence.You can contact them directly or referrals can be made by nursing staff, family, friends and carers. Their scheme is to complement, not replace, any other care provided by family, friends or other agencies.All volunteers will carry identification cards and will have been introduced to you by the co-ordinators so that you will know who it is that will visiting you.The criteria for the service are that service users are 60 or over and live in south Shropshire.For more information, please follow the link to visit their webpage, or contact using the details provided.
