Dementia care across Shropshire is provided by the Midlands Partnership NHS Foundation Trust (MPFT). MPFT provides a memory and dementia care pathway across South Staffordshire, Shropshire and Telford & Wrekin that provides specialist help for people with memory problems with a diagnosis of dementia, and their family and carers. The service offers specialist memory assessment, diagnosis and treatment. MPFT also provide advice and information for the individual and anyone who cares for them. The aim is to avoid hospital admission wherever possible. An Access Team provides a single point of contact for all the Trust’s adult mental health services, including people with dementia.
